Rights and Responsibilities
Failure to read this information does not excuse students from the rules and procedures described herein. Personal factors, illness, or contradictory advice from any source are not acceptable grounds for seeking exemption from these rules and procedures. The university reserves the right to deny admission to any applicant, to discontinue the enrollment of any student whose personal actions are detrimental to the university community or are in violation of any applicable code of conduct, and to require the withdrawal of a student whose continuance at the university would be detrimental to himself or herself or to others. It is the responsibility of students to become familiar with the academic regulations and procedures relating to their courses of study at the university.
Right to Privacy
Every student has the right to request that the university not release directory information, including address, telephone number, or dates of attendance for any purpose without accompanying authorization from the student. The right to be excluded from the printed Student Directory must be renewed each term, during the first two weeks of classes. Requests may be made weekdays in the Office of the University Registrar, B7 Day Hall. You may also view the Cornell Policy on Access to and Release of Student Education Records.

Immunization Requirement
All students who register for six or more credits a semester must complete Gannett's online health history form, which can be found at forms.newstudents.cornell.edu. Students must provide proof of immunization against tetanus, measles, mumps, and rubella. It is also mandatory for all international students (excluding those from Canada), regardless of the number of credits, to receive a tuberculin skin test (PPD, Mantoux) within three weeks of arriving at Cornell.
Please note: Students who fail to complete their health requirements will be assessed a $100 penalty. Additionally, New York State requires the cancellation of the registration of any extramural study student taking six or more credits who has not presented proof of the required immunizations.
