The information presented below pertains to the 2008 Summer College program. We're currently putting the finishing touches on our 2009 program and we'll update the Web site with this information by mid-December. If you would like to be notified when the site is updated, please join the Summer College e-mail announcement list.

Paperwork checklist

Please be sure to read this page in its entirety.

The full program charge is due upon acceptance to secure registration in the program and course(s) for which your student has been accepted. Only students who are paid-in-full are assigned housing.

Please return the following forms as soon as they are completed and no later than June 13 (except where mentioned below). If you cannot meet the deadlines, please bring the completed forms with you on Opening Day. If at all possible, we recommend that you make a copy of the completed forms before mailing them and bring the copies with you on Opening Day.

Note: If you wait until Opening Day to return the forms, you will be delayed during check-in. Also, your student's Cornell ID card will not be ready for you, which is a great inconvenience in gaining access to your student's residence hall and eating in the dining hall.

We can't stress strongly enough how important it is to return these forms to us as soon as possible so we can provide your student with timely medical care (if needed); allow her/him to check into her/his room, eat in the dining hall, participate in field trips; and give her/him access to laundry services. You must complete the following three forms and return them to Summer College by June 13:

  1. Code of Conduct Form
    Please review and discuss this form with your student. We are very serious about students maintaining the academic and social standards outlined in this document.
  2. Cornell ID Card Request Form
    Students use their ID card to access numerous services, most importantly to gain entry to their residence hall and eat in the dining hall.
  3. Permission Form
    This form gives your student permission to participate in field trips and athletics. It also gives Summer College permission to release images of your student.

Once completed, mail the above three forms to:

Please complete the following two forms and return them to Gannett Health Center by May 16:

  1. Health History Form (This form is required)
    Gannett Health Center must have a completed and signed form on file in order for your student to receive timely medical attention, if necessary. This form usually takes the longest to complete; we suggest that you make an appointment with your health care professional as soon as possible to obtain the required information and signature. If you have questions about the form, please call 607.255.4364 or 607.255.6951.

    On the form, you will need to note how many credits your student is taking. Please refer to your student's Enrollment Form, which was mailed with the Acceptance Packet, for this information. If you have questions about the number of credits, please call 607.255.6203.

  2. Health Information to Assist Senior Residential Staff and FacultyComplete only if the conditions on the form apply to your student.
    We request that you provide the Summer College senior residential staff and faculty of programs that take place off campus with some basic information about your student's health. This will enable us to work with you to accommodate special needs and provide assistance, if necessary.   1. Credit Card Form
    You need to complete this form only if you are paying the program charge by credit card.

    Once completed, mail or fax—do not do both—the above form to:

  2. Student Health Insurance Plan (SHIP) Enrollment Form (Due by June 1.)
    You need to complete this form only if, after reviewing your own health insurance plan, you decide to enroll your student in the SHIP. If you have any questions, please call 607.255.6363 or e-mail sicu@cornell.edu.

    Once completed, mail the above form to:

  3. Request for Disability Services and Accommodations Form
    You need to complete this form only if you are requesting specific academic, housing and/or transportation accommodations due to a disability or medical condition. (See #4 below under Helpful Hints.) There is no specific due date for this form but requests must be made in advance of anticipated need.

    Once completed, mail or fax the above form along with disability documentation to:

  1. Review the schedules
    Before making travel plans, read the Opening Day and End-of-program checkout dates and procedures.
  2. Coordinate travel to and from campus
    If your student is traveling alone, you’ll need to schedule transportation to and from campus.
  3. What to bring
    This is a very helpful list, compiled by Summer College alums of recommended items to bring.
  4. Disability services are available for Summer College students
    If your son or daughter receives disability services in high school, requires specific housing arrangements for disability or health reasons, or needs transportation assistance, please contact Student Disability Services at 607.254.4545 or email clt_sds-mailbox@cornell.edu. Students who wish to use disability services must submit disability documentation and discuss accommodation requests with SDS staff in advance of need.
