First things first

Pay program charge

The total program charge is due in our office within 5 business days from the date of your acceptance letter. If you are admitted within a week before or at the application deadline, full payment is due the day of your acceptance. Students are NOT enrolled in their classes or assigned housing until the total program charge is paid in full.

Activate NetID and Cornell e-mail address

Instructions on how to activate your NetID will be sent only to you (the student) via e-mail from Cornell Information Technologies (CIT) starting late-March/early-April and within a week after your program charge is fully paid. Be sure to follow the steps included in the message.

Your Cornell NetID is your key to secure online services at Cornell. You are expected to activate your NetID and Cornell e-mail account immediately upon receipt.

Your NetID must be activated before you can access and complete the Health History Form and submit the Code of Conduct and picture for your Cornell ID Card. (See below.)

Rent a refrigerator and/or extra fan (optional)

Here's the fine print on equipment rentals: Campus Life tries to ensure that the rental agency provides quality equipment and reliable service, but the university cannot be held responsible for contracts made by students.

We encourage you to submit your order before arriving on campus. Refrigerator and fan rentals are also available on Opening Day.

Familiarize yourself with bursar accounts

For your information, Cornell will create a "bursar account" for Summer College students who incur charges from Cornell departments such as Gannett Health Services, the libraries, and Cornell Information Technologies (CIT). If your student incurs these charges, s/he will receive a bursar bill. Bursar bills are ONLY sent electronically. Bursar accounts will also be used for any additional residential charges such as early arrival/late departure costs or billing for damages to residence hall rooms or common areas. Students with room damage will get an additional e-mail with information on the damage and instructions on how to appeal. Appeals are due by September 30, 2016. Notifications of bursar charges are sent to the students' Cornell e-mail address. The e-mail will contain a link to the University Bursar's office through which you can obtain instructions about how to view and pay these charges through a Cornell CASHNet account. Note: If full payment is not received by the due date, a late-payment penalty (finance charge) of 1.25 percent of the past-due amount will be assessed. Unpaid accounts are forwarded to Cornell's Collections Office ninety days after their due date. Call 607.255.2336 or e-mail uco-bursar@cornell.edu if you have questions regarding a bursar bill.

Forms to fill out

We have sent you an e-mail with instructions about how to use our online Student Services system, including where you can check the status of your forms.

Note: If you wait until Opening Day to submit any required forms, you will be delayed during check-in.

Code of Conduct: To be completed by the student
Submit as soon as possible, and no later than June 10 (NetID required)

The Code of Conduct will be available in mid-May.

Parental Permission and Acknowledgment Form: To be completed by the parent/guardian
Return as soon as possible, and no later than May 27

This form asks you to grant (or withhold) permission for your child/ward to participate in travel and/or athletic activities and for Cornell to use your child's/ward's likeness, image, and/or voice in promotional, informational, or other materials.

(Note: The photos of students who participate in the Summer College group photo sessions, organized by residential floors, will be included in the Summer College yearbook. If you withhold permission for Cornell to use your student's image, please remind your student to excuse her/himself from these photo sessions.)

Students cannot travel or participate in activities until this signed form is on file. Please return this form in one of the three ways (mail, e-mail, or fax) listed at the top of the form.

Cornell ID Card Request Form
Submit as soon as possible, and no later than May 27 (NetID required)

You are required to submit a photo online for your Student ID Card. Your ID card can be used as an electronic door key and to access a variety of facilities on campus, including, dining, laundry, fitness centers, and libraries.

Health History and Immunization Compliance
Submit as soon as possible, and no later than May 16 (NetID required)

Completing this form can take several weeks, so you should log in and review the instructions as soon as possible (after your program charge is fully paid and you've activated your NetID). Note that the form must include documentation that you have received all required immunizations.

To avoid late fees and registration delays, your printed Health History Form must be postmarked by May 16, 2016.

All questions regarding the Health History Form go directly to Gannett by e-mail at immunization@cornell.edu or by phone at 607.255.4364.

Steps:

  1. Log in to the "Health History Form" link below and review the complete instructions*
  2. Fill out form online, using your immunization records, if you have them, submit the form, and print it
  3. If you need to see your health care provider, bring the form to the appointment for information and signatures
  4. Make sure to provide all required parent/guardian signatures on the printed form
  5. Mail the signed form by May 16 (and attach your immunization records, if applicable). Students who are accepted after April 29th have 10 days to complete the form to ensure the form will be processed before any financial penalties or disruptions to your student status.
  6. Check your updated compliance status on your Student Services account.

Health Information Form (optional)
Return as soon as possible, and no later than May 16

This form assists the senior Residential Staff and Faculty. Complete only if the conditions on the form apply to your student.

Student Health Plan (SHP) (optional)
Contact Summer College no later than May 27

Review the requirements on our Student health coverage page and compare the Cornell Student Health Plan with your own health insurance plan. If you decide to enroll your student in the SHP, contact Summer College no later than May 27.

Disability Self-Disclosure and Request for Access Accommodations form (required for students requesting disability services)

If your student has a disability and will need disability services to participate fully in the Summer College Program, please submit a Disability Self-Disclosure and Request for Access Accommodations form online. (Click on "Register with SDS" to access the form.) If you have questions, please contact Student Disability Services at 607.254.4545 or sds_cu@cornell.edu.

This form will be available beginning March 1, 2016. Once you have activated your Cornell NetID, you can complete and submit this form. It must be completed in advance of starting classes. Note that it can take up to two weeks to evaluate and determine eligibility for disability accommodations. Some accommodations, such as specific housing, alternative formats for materials/textbooks, or interpreters, require more advanced notice than others.

Permission Form for Riding in a Private Car (optional)

If your student will be riding in a private car, you will need to submit this permission form.

This form may be submitted after your student arrives on campus and by 4:00 p.m. on the day of departure or by 4:00 p.m. on Thursday for the weekend. Permission forms are not accepted on Fridays, Saturdays, or Sundays.

Permission Form for Staying Overnight Off Campus (optional)

If your student will be staying overnight off campus, you will need to submit this permission form.

This form may be submitted after your student arrives on campus and by 4:00 p.m. on the day of departure or by 4:00 p.m. on Thursday for the weekend. Permission forms are not accepted on Fridays, Saturdays, or Sundays.

Register your bike (optional) (NetID required)

If you bring a bike with you, you are required to register it as soon as you get to campus. The form requires the bike's serial number.

Other information you will need to provide on the form is the make, model, colors, and style of the bike. We also encourage you to include your first and last name, a phone number and e-mail address to contact you in case you need to identify your bike or there is a problem with it.
